This notice explains how the appointment form on this website handles information. Confirm the final wording with the clinic and adapt it to applicable Bangladesh law before publishing.

What information is collected?

If you submit an appointment request, the website stores your name, phone number, selected appointment date and time, booking status, and the time the record was created. The form does not ask for symptoms, diagnosis, scans or other clinical details.

Why is it collected?

These details are used to reserve and manage the requested appointment and to contact you if needed. The clinic should only allow authorised staff to access the records.

Where is it stored?

Appointment records are stored in a Cloudflare D1 database configured for this website. The administrator area is password-protected. Website operators should protect secrets, restrict database access, enable suitable rate limiting, and review Cloudflare's applicable data-processing terms.

Retention and deletion

The website does not currently delete records automatically. The clinic administrator must adopt a retention period and securely delete records when they are no longer needed, subject to applicable recordkeeping requirements.

Your choices

Do not enter medical details in the booking form. To request a correction or deletion, contact the clinic at +880 1711-153692. Avoid sending sensitive health information over ordinary messaging.

Medical emergency

This form is not monitored as an emergency service. For urgent or severe symptoms, go to the nearest emergency department.
